Beschreibung

My girlfriend from Brazil was visiting Germany over Christmas and had booked her return flight for early February.

On 11.01.2026 we were together at a skate park nearby. It was Sunday morning and there was practically no one around yet. After a short time on the skateboard, she fell completely unexpectedly onto her left wrist and broke her radius. For both of us, this moment was a classic „textbook“ DHS (unexpected, acute, dramatic) - the wrist was visibly broken, and we drove straight to the nearest university hospital.

There, her hand was put back into position, put in a cast, and a surgery appointment was scheduled for the coming days. Since my girlfriend unfortunately had no travel health insurance and was only visiting Germany, we would have had to pay the costs of the operation ourselves, in the four-figure range. Because of the high costs of private treatment without insurance, three days later, on 14.01.26, we decided to rebook the flight (resolution) and have the surgery done in Brazil instead, since my girlfriend was adequately insured there.

That was definitely an important conflict resolution, and that night we both sweated very heavily (the bed was completely soaked). The flight was then two days later, on 16.01.26 at 8 a.m., and my girlfriend arrived in Brazil „safe and sound“ with her broken arm and was able to have surgery there the following week.

During the night of 17-18.01.26, I woke up at 3 a.m. with very severe pain in the upper abdominal area. It was very sudden and unexpected (epi-crisis), since I had actually been sleeping well and had had no abdominal complaints at all when falling asleep. There was strong cramping in the upper abdomen/lower rib cage; the pain was continuous and strong (no intervals); the muscles were very tense and stiff, almost feeling like a severe case of muscle soreness. I found it hard to sit upright and my breathing was severely restricted. This lasted about 40 minutes and then suddenly weakened, letting me fall back asleep about 1 hour after it started. Taking Bitterstern drops helped me a lot. The next day there were no symptoms left at all.

A possible explanation would be an SBS of the striated musculature in the area of the diaphragm or abdomen/intercostal muscles, with the conflict content "I can't manage this", "I am physically overwhelmed", "unable to flee". Of course, several conflicts occurred during that week, which can make a clear assignment difficult. Fortunately, my girlfriend is doing well again; she can already move her fingers/wrist again and is currently enjoying carnival in Brazil 😉

The conflict shock (DHS) was on 11.01 -> resolution 4 days later by rebooking the flight on 14.01 -> PCL-A without symptoms, with interruptions, until 17.01 -> EK (epi-crisis) during the night -> Then PCL-B without symptoms.

Note from Ingmar Marquardt:

Thank you very much for this instructive report - we discussed this case together in our study group. Regarding the distinction:

In terms of location, I would expect pain from the intercostal musculature to be higher up, i.e. really primarily between the ribs and more to the side.
The situation, as you described it in the report and in the study group, fits very well with the conflict perception of the diaphragm, i.e. "I can't catch my breath", "I'm given no room to breathe" - until finally, after several days of extreme tension with various appointments and phone calls, the resolution of the difficult situation was finally settled.
